چکیده
Patients with cardiogenic shock (CS) requiring mechanical circulatory support (MCS) are complex. The Impella CP (Abiomed, Danvers, MA) is a percutaneously placed left ventricular assist device and there no established best-practice for removal. Vascular complications from MCS devices common many patients undergo operative removal of these devices. To our knowledge published data concerning the safety bedside femorally-placed in cardiovascular intensive care unit (CICU) CS. We conducted retrospective chart review all CS at institution 2020, approved by institutional board. In 2020 we cared 84 CICU, excluding intra-aortic balloon pumps. Of these, 55 had CP, either alone or veno-arterial extracorporeal membrane oxygenation. 18 died place three were transferred to another facility on MCS. 14 their removed operating room five cardiac catheterization lab. 15 total (27%) underwent CICU. removal, vascular closure was used vast majority (87%). Only one patient post-removal leg ischemia surgical endovascular intervention; this preexisting peripheral disease via manual pressure clinical ischemia. Bedside feasible safe select patients. More research needed establish best-practices

Cardiogenic shock: From ECMO to Impella and beyond.
For patients in cardiogenic shock, several devices can serve as a "bridge," ie, provide circulatory support and allow the patient to live long enough to recover or to receive a heart transplant or a long-term device. Options include an intra-aortic balloon pump, TandemHeart, Impella, extracorporeal membrane oxygenation (ECMO), and CentriMag. Which device to use depends on individual patient nee...
متن کاملTreating Refractory Cardiogenic Shock With the TandemHeart and Impella Devices: A Single Center Experience
BACKGROUND Patients with cardiogenic shock (CS) are routinely treated with intra-aortic balloon pumps (IABPs). The utility of 2 new percutaneous left ventricular assist devices (PLVADs), the Impella and TandemHeart, is unknown. The objective of this study was to describe the use of PLVADs for patients with CS at our institution. METHODS All cases involving PLVADs in patients with CS between b...
متن کاملManagement and outcome of patients supported with Impella 5.0 for refractory cardiogenic shock
INTRODUCTION Cardiogenic shock refractory to standard therapy with inotropes and/or intra-aortic balloon pump is accompanied with an unacceptable high mortality. Percutaneous left ventricular assist devices may provide a survival benefit for these very sick patients. In this study, we describe our experience with the Impella 5.0 device used in the setting of refractory cardiogenic shock. METH...

OBJECTIVE To determine whether early activity is feasible and safe in respiratory failure patients. DESIGN Prospective cohort study. SETTING From June 1, 2003, through December 31, 2003, we assessed safety and feasibility of early activity in all consecutive respiratory failure patients who required mechanical ventilation for >4 days admitted to our respiratory intensive care unit (RICU). A...
متن کاملThe contemporary role of Impella in a comprehensive mechanical circulatory support program: a single institutional experience
BACKGROUND The treatment of cardiogenic shock with percutaneous mechanical circulatory support (MCS) is attractive: however, at present it is not clear which is the best strategy, as no survival benefit has been demonstrated for any device as single therapy. Aim of this study is to describe the role of percutaneous Impella in a comprehensive MCS program. METHODS Observational study on 22 pati...
